For some transgender, nonbinary, and gender nonconforming people, facial feminization surgery is a vital part of treating gender dysphoria and their gender transition care plan. Not all transgender women will seek out FFS or any gender affirming procedure, however, for those that do, it can be a lifesaving procedure. FFS can positively impact the patient’s mental health and well being. While Dr. Sajan does work with insurance for facial feminization, patients can also pay out-of-pocket for a reduced rate. Out-of-pocket patients are not required to provide such letters, however, insurance patients are required by the insurance carriers to provide mental health support letters.

All packages include fees associated with the surgeon, anesthesia, operating room facility fee, bandages, and facial bra (compression garment). Also, the below packages are not the only possible facial feminization surgeries available. Dr. Sajan customizes each procedure to the patient and their goals and patients can modify their surgical plan, where appropriate. The prices include surgically removing the forehead skin and advancing it, if you desire an immediate hair transplant at the same time as FFS, additional costs will apply. We are one of the only surgical practices in the world that offer immediate hair transplantation at the same time as your FFS.

An Overview Of Facial Feminization Procedures
An FFS procedure can involve a combination of several procedures. Therefore, each facial feminization surgery can look completely different. Not all patients will need all of the procedures listed below, but they include the most common procedures Dr. Sajan performs during FFS.
People assigned female at birth tend to have lower hairlines and unlike men, when they lose hair, it normally does not involve a receding hairline. Thus, during hairline lowering Dr. Sajan will essentially create a new hairline lower on the head and remove the skin between the current hairline and the new one. Occasionally, hair transplantation is also involved for the best possible result.
Along with hairline lowering, some patients prefer a hair transplant to create a more feminine hairline and support long lasting results. This is not necessary for all patients, but it can help round out the hairline and sometimes replaces the need for a hairline lowering procedure.
The brow bone sits roughly even with the eyebrows. However, people assigned male at birth have brow bones that protrude further outward whereas people assigned female at birth have a more naturally sloping forehead without an overly prominent brow ridge. During a brow bone reduction, Dr. Sajan reduces the protrusion of the brow bones to achieve a more feminine appearance to the upper face. He also places titanium plates to keep the reconstructed frontal bone in place and maintain results.
Orbital Bone Reduction
The orbital bones—which make up the eye socket—also tend to protrude forward on a male face. An orbital bone reduction reduces the protrusion of the upper portion of the orbital bone structures. This is frequently done with a brow bone reduction to create a perfectly smooth contour.
Traditionally, a brow lift is an anti-aging surgery. However, there are some cases where the eyebrows may require a slight lift even without a brow bone reduction. This can occur with or without a brow bone reduction. Unlike the version of a brow lift that corrects aging, this brow lift generally removes less skin.
Rhinoplasty
A nose job can make up an important part of FFS for some patients. Men usually have bigger noses than women. Dorsal humps—humps on the bridge of the nose—are more common in men. Thus, during an FFS, rhinoplasty can play an important role in overall feminizing the face.
Midface Lift
A midface lift elevates the cheeks to accentuate the natural structure of the face. Women tend to have larger cheeks than men. Therefore, lifting the fat of the cheeks and removing skin strategically can create a more feminine and defined cheek appearance.
A lip lift is a procedure that reduces the distance between the upper lip and the nose. While there are only a few millimeters of difference between men and women in this area, lifting the lip upward can help feminize the lips and lower face. Additionally, it may help enhance the shape of the top lip.
Mandible Chin Bone Reduction
The ideal male and female chins both tend to fall in line with the rest of their features. However, in general, men have bigger chins and with the reduction of other features such as the brow bones and nose, the chin may need adjusting as well. During a mandible chin bone reduction, Dr. Sajan shaves down the chin bone to better reflect the rest of the patient’s facial features.
Jaw shape is a major difference in gender expression. Men prefer jaws that may protrude outward more and take on a straight, square shape. A woman’s jaw is subtler and smaller with a soft curve down to the chin. Dr. Sajan may shave down wide jaw bones to take on a more feminine appearance.
The Adam’s apple is a structure that males have that is absent in cisgender females. While the structure cannot be safely removed during facial feminization surgery, Dr. Sajan can use a tiny drill to gently shave down the cartilage to reduce the appearance of the Adam’s apple.
