Dr. Sajan will address specific parts of the body to help you achieve a feminine, hourglass shape.
What
is body feminization?
Body feminization is a combination of surgeries–performed separately or together–that help people transitioning from male-to-female achieve body contours and proportions that better align with the female body. Every body feminization treatment plan is customized for each individual transgender person. The goal is to help alleviate gender dysphoria which is generally defined as an incongruence between the inner and outer self based around gender and the person’s gender assigned at birth. Dr. Sajan will help sculpt a narrower waist, shapelier buttocks and hips, and an overall more feminine body appearance.
Dr. Javad Sajan is a Seattle-based, board-certified plastic surgeon dedicated to cosmetic and reconstructive surgery of the face and body. His practice has treated patients from across the United States and around the world, and he brings deep experience in gender-affirming procedures, including body contouring for transgender patients. Dr. Sajan’s approach is grounded in personalized care, safety, and outcomes that reflect each patient’s unique goals.
How is Body Feminization Done?
In nearly all body feminization procedures that Dr. Sajan performs he uses liposuction and fat transfer. Commonly, fat is taken from areas such as the waist, thighs, and abdomen; then, transferred into areas such as the buttocks, hips, or breasts. To minimize trauma and reduce recovery time, he uses power-assisted liposuction and J plasma techniques to achieve the following:
- Remove fat from places where males traditionally carry fat
- Transfer fat to the hips and buttocks to create a curving, attractive contour.
Sculpt a slimmer, more narrow waist. - Create a slim hourglass figure.
- Tighten skin over areas where fat was removed.

Consultation
Dr. Sajan will begin with a thoughtful, thorough consultation to understand your body transitioning goals and ideals, your concerns, and your medical history. He will examine your body, skin, and muscle tone, and ask about any drug sensitivities, your lifestyle, and daily habits.
Once the consultation is complete, Dr. Sajan will develop a surgical plan customized specifically for your needs to help achieve your aesthetic goals. Your surgical procedure will vary based on what the surgical plan is for you exactly.
Procedure
In general, your body feminization procedure will begin with general anesthesia. To achieve the slim, hourglass figure that is widely regarded as the classic sign of feminine beauty, Dr. Sajan will design the ideal waist-to-hip ratio, which is determined by dividing your waist measurement by your hip measurement.
Dr. Sajan will perform gentle liposuction in the waist area. He will begin with a micro-incision, insert a fine tube called a cannula, and use high-definition liposuction to remove fat from your midsection in order to sculpt the slim, small waistline you want. He will then purify and cleanse this fat so that it can be transferred into the buttocks and hips.
Dr. Sajan will inject your body fat at strategic points throughout the buttocks and hips, creating a more curving, rounded, feminine contour. Breast augmentation with fat transfer can be a part of this procedure, if your breasts haven’t already been created with implants.
Patients are often worried about scars after the surgery. Dr. Sajan performs body feminization through tiny liposuction entry points that are placed in discreet locations, such as the areas covered by underwear. Over time, these marks generally fade away and become less noticeable. During your consultation, you will receive a full explanation from Dr. Sajan on scar placement and scar care.
Recovery after body feminization
During your initial recovery period, you will wear a compression garment to reduce swelling and support the relocated fat tissue. Expect that there will be bruising and swelling in the donor and recipient sites. You will likely want to spend the first few days of the recovery period resting, but do be sure to begin walking around the house as soon as possible to promote healing.
Patients usually return to work after about one week. Do not engage in strenuous exercise or heavy lifting for several weeks until the healing process is complete.
Results
The new, feminine body contours will be noticeable almost immediately. As the swelling reduces, your results will continue to become increasingly evident over the following weeks.

FAQ
Are There Non-Surgical Hairline Feminization Options?
Unfortunately, there are not any effective non-surgical hairline feminization procedures. Certain non-surgical hair restoration methods can help, but they cannot restore or build a hairline like hairline advancement or hair transplant. Patients can use non-surgical hair restoration procedures to supplement and maintain results, but surgery is needed to produce the optimal results of hairline feminization surgery.
Does Insurance Cover Hairline Feminization?
Yes, insurance will often cover transgender hair transplant and hairline surgeries. We accept all private insurances.
What Shape Will My Feminized Hairline Be?
Without seeing a patient’s hairline, it is impossible to determine the shape of their feminized hairline. In general, a feminine hairline will look more rounder than a masculine one. The exact height and shape of the hairline will mimic the current one. If a patient has a high hairline, the feminized version may remain high.
Certain anatomical factors such as scalp laxity also matter. If the scalp is already quite tight and there is not much room to lower the hairline, Dr. Sajan may use a specialized tissue expander to slowly expand the skin of the scalp. This will allow for better results. Most patients will not require this.
Upon consultation, Dr. Sajan will review the patient’s goals and what they want to achieve from hairline feminization surgery. He will craft a customized surgical plan for each indivudal patient. This helps ensure he maintains their unique features (if desired) and that the procedure is done safely.
Can You Have Hairline Feminization During FFS?
Yes, hairline feminization is frequently performed during facial feminization surgery. = Dr. Sajan is one of the few surgeons who will perform a hair transplant procedure at the same time as FFS. The hairline lowering procedure, though, is commonly seen during FFS.
Are The Results Permanent?
Yes and no. It depends on the specifics of the procedure and the patient. For example, hairline advancement is usually permanent. Though, hair transplant may still fall out due to age. However, it is should be noted that since testosterone is often reduced in transgender women, this makes the hair much less susceptible to falling out. Patients can expect their hairline feminization results to age with them and they tend to age well.

Body feminization is a set of gender-affirming surgeries designed to transform masculine body features, such as the hips, stomach, thighs, and buttocks, into a feminine contour. Your plan can be done in one surgery or staged, depending on your goals and anatomy.
Most body feminization surgeries use liposuction and fat transfers. Fat is commonly removed from areas such as the abdomen, waist, or thighs. This is then transferred to the hips, buttocks, and, in some cases, the breasts. The exact combination is customized to create a smoother waist-to-hip transition and a more hourglass figure.
While recovery times vary, most patients are able to return to work in about one week. You should anticipate bruising and swelling, and you will need to wear a compression garment. Light walking is encouraged soon after the procedure. However, strenuous exercise and heavy lifting must be avoided for several weeks until your surgeon gives you clearance.
Yes, most results can be long-lasting, especially if you have a stable weight. During the fat transferring process, it becomes a part of your body, keeping the shape and getting refined over the next few weeks of the healing process.
Yes, but the scars are tiny. These are well hidden under the underwear. Dr. Sajan makes precise, small incisions for liposuction and recommends optimal scar-recovery creams to minimize scarring.
